NEBAA/USABA Sports Education Camp

Dear Parents and Athletes,

Thank you for your application to Sports Camp 2012! Congratulations! You have been accepted to the camp that is being held on the campus of the University of Maine in Orono from Wednesday June 20 – 23rd, 2012.

Please contact Mark Sinclair about transportation to Sports Education Camp. 

Here are the forms you need for the SPORTS EDUCATION CAMP. Forms 1-4 need to be completed and submitted by June 11th. Form 5 is for your use only. Download PDF Package of all the forms, or fill out the forms individually by clicking the links below.

  1. WAIVER/MEDICAL FORM – this is the agreement, assumption of risk, release, waiver & consent form. It is a liability release allowing us to photograph your child and to take him/her to the doctor if needed. To be filled out by parent.

  2. CODE OF CONDUCT FORM – expectations of proper athlete behavior to be signed by Athlete

  3. USABA APPLICATION – 2 page application form. Your athlete must be an active member of USABA in order to be covered by insurance for this camp. Please submit your application with the $25 dollar check made out to U.S.A.B.A. If your athlete is already a 2011 member of USABA, return the form with this noted

  5. SCHEDULE OF EVENTS

The first four of these documents must be filled out and returned with a $25.00 check or money order, payable to USABA. If these forms cannot be returned within the time frame allowed, please let Andrea know by calling 944-3754 and other arrangements may be made.

The athletes and chaperones will be staying in a dorm on the campus of U of M. There will be at least one adult supervisor for every three athletes. Linens, pillows, meals, and snacks will be provided, but you must supply personal items such as soap, shampoo, and change for vending machines or phone calls. The week’s activities will be athletic; so all athletes are required to bring sweat pants/shirts, shorts, t-shirts, athletic socks, swimsuit, towel, gym shoes and a hat with a brim. A windbreaker or raincoat is recommended. A backpack or gym bag would also be needed to carry gear each day. Please attach name labels to all items, including luggage. Please do not send electronics, or other valuables, as we cannot be responsible for them.

Please note the enclosed schedule. Some important times and information:

  • Check-in: Wednesday, June 20, 2012 , 12 noon at (location TBA). Note: please bring any medications that your child needs in the original containers and properly labeled.

  • Awards Ceremony and Pizza: for parents & athletes – Saturday, June 23, 2012, 12:30 – 1:30 p.m.

  • Final Pick up Saturday, June 23 at 1:30 (note: your child must be picked up no later than 1:30 pm on Saturday. There will be no staff available after that time)

  • 8th Annual Maine State Games for Youths with Visual Impairments

    • TBA - Wrestling

    • Saturday, Time TBA - Goal ball

    • Saturday, Time TBA - Track and Field

Note: Parents, teachers, and friends are invited to the 8th Annual Maine State Games for Youths with Visual impairments to watch as your athlete sails as they do the long jump, waits anxiously for the exchange in a relay race, or play in a team sport. If you are going to join us for the state games, you can stay in the dorm for a reasonable price or any area motel. Please inform Mark Sinclair if you would like a room on Friday night in the dormitory.

There is no charge for the camp itself. However, we spend approximately 200.00 per athlete for room & board and would welcome donations of any amount. If you would like to make a donation, please make checks payable to USABA with the forms you are sending back to us. We do require that each athlete join the USABA, which costs $25.00. The USABA helps support the camp, coordinates competitions for blind athletes, and will send a newsletter to your son or daughter.

  • Check-in: Friday, March 9th at 8:00 a.m. at the Maine Adaptive Sports and Recreation Lodge at Sunday River. Please bring any medications that your child needs in the original containers and properly labeled. NOTE: If the athlete is on medication please provide 2 extra days worth if medication in case of snow storm or other inclement weather.

  • Awards Ceremony and Pizza: for parents & athletes – Sunday, March 11th at 1:30.

  • Final Pick up Sunday, March 11th at 1:30 (note: your child must be picked up no later than 1:30 pm on Sunday. There will be no staff available after that time)

  • 1st Annual Maine State Games for Youths with Visual Impairments

    • Sunday- All competitive sports.

Note: Parents, teachers, and friends are invited to the 1st Annual Maine State Winter Games for Youths with Visual impairments to watch as your athlete participates in winter competitive sports. Please inform Mark Sinclair if you would like a room on Friday night in the dormitory.
